DescriptionThe Care Manager MSW partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The Care Manager MSW is responsible for providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral.

Required Qualifications
Master's Degree - In Social Work or related field from an accredited educational institution.
Preferred Qualifications
1 year - Recent inpatient clinical experience in an acute care facility.
1 year - Brief intervention/therapy/counseling in a hospital setting
